Many companies which have successful Lean Manufacturing programs, use a 5 Interrogative approach for technical problem solving. By the end of the year, the result is a “problem”. This can be a very powerful way of doing it. However, there are two major challenges with the 5 Why approach; The first is the answer to the question. Knowing and perceiving are not the same thing. The second is how the question is phrased. A subtle change in the wording will lead the team in a completely different direction.
